  1. 1Choose a healthcare advocate (durable power of attorney for healthcare).
  2. 2Complete a Washington State Advance Directive or Five Wishes document.
  3. 3Discuss POLST (Physician Orders for Life-Sustaining Treatment) with your doctor.
  4. 4Gather important documents: ID, insurance, will, financial accounts, passwords.
  5. 5Write down preferences for the room: music, lighting, scents, who to call.
  6. 6Talk through funeral, burial, or cremation wishes — and who will carry them out.
  7. 7Consider a legacy project: letters, voice recordings, photo albums.
  8. 8Make a list of people to be notified, with phone numbers.
  9. 9Identify spiritual or cultural practices that matter to you or your loved one.
  10. 10Share copies of completed documents with family, your doctor, and your advocate.
